Our Chronic Care Program helps members with chronic conditions better manage their health and improve their quality of life. These programs, which are offered at no cost to eligible members, reinforce your doctor’s treatment plan through educational mailings and support from our Care Managers, who are all registered nurses. The program is available for those members with:
Our Asthma Chronic Care Program helps you understand the proper use of metered dose inhalers and quick-relief medications, and how to identify and avoid your unique asthma triggers. We encourage you to talk with your physician to create an individualized asthma action plan. This program is available to eligible Horizon BCBS members who are at least five years old and diagnosed with asthma.
Chronic Obstructive Pulmonary Disease (COPD)
Our COPD Chronic Care Program is designed for those with airflow obstruction due to chronic bronchitis and/or emphysema. We can help you learn to identify the signs and symptoms of an impending COPD exacerbation, and help you understand the importance of medication management. This program is available to eligible Horizon BCBS members who are at least 30 years old and diagnosed with COPD (includes chronic bronchitis and emphysema).
Coronary Artery Disease (CAD)
Our CAD Chronic Care Program helps you understand the importance of taking appropriate medications for your condition and the importance of exercising, eating right and other heart-healthy lifestyle changes. This program is available to eligible Horizon BCBS members who are at least 18 years old and have been hospitalized for cardiovascular conditions like angina or a heart attack (acute myocardial infarction) or had procedures such as coronary artery bypass graft (also known as CABG) surgery, percutaneous transluminal coronary angioplasty or stent placement.
Our Diabetes Chronic Care Program helps you understand the importance of your medication, routine self-monitoring of your blood glucose levels, good nutrition, exercise and stress reduction to control your diabetes. We provide reminders for important monitoring tests, including dilated eye exams, foot examinations, A1C, LDL and microalbuminuria testing. This program is available to eligible Horizon BCBS members of all ages diagnosed with diabetes.
Our Heart Failure Chronic Care Program helps you manage your condition by providing education to supplement your physician’s plan of care. Although there is no known cure for heart failure, our program can help you better understand and monitor the symptoms. This program is available to eligible Horizon BCBS members who are at least 18 years old and diagnosed with heart failure.
Our comprehensive Chronic Care Program offers eligible members:
- A Care Manager with expertise in your condition
- Access to a team of health professionals, including a registered dietician, social worker, behavioral health specialist and pharmacist
- Assistance in navigating the health care system, understanding your benefits, and getting the appropriate care when you need it
- Quarterly educational mailings
Eligible Horizon BCBS members can enroll in the Chronic Care Program online or by calling 1-888-357-3515. Assistance is available Monday through Friday, from 8 a.m. to 7 p.m. Eastern Time.
