Horizon Dental Choice (HDC) Plan S is the Dental Plan Organization (DPO) offered by Horizon Blue Cross Blue Shield of New Jersey to state government employees and retirees and to certain local government and local education employer groups.
HDC covers 100 percent of all eligible preventive services and most basic dental services with no copayments, maximums or deductible when a primary care dentist in the HDC network provides the services. HDC also covers charges for all eligible major and specialty services with affordable copayment amounts when services are referred by your primary care dentist to a Horizon PPO Dental specialist. There is no out-of-network benefit.
Members must choose a primary care dentist from the HDC network and can change the primary care dentist monthly, if they wish, effective on the first day of any month by giving Horizon BCBSNJ 15 days’ notice.
To find dentists in the HDC network, use the Doctor & Hospital Finder and choose Horizon Dental Choice as the plan name to find a participating dentist near you, including detailed door-to-door directions and a street map.
Prevention is the best defense against dental disease. Once you’re enrolled, be sure to schedule your appointment for a checkup.

There are no out-of-network benefits in the HDC plan. You must use a dentist in the HDC Network to have coverage. Specialty dental care requires a referral from your PDC; specialists are in the Horizon Dental PPO Network.
Yes, your PDC must give you a referral to see an in-network specialist. Please call Dental Customer Service at 1-800-4-DENTAL (433-6825) for help selecting a specialist in the Horizon Dental PPO Network.
If your current dentist is not in the HDC Network, tell us your dentist’s name and we will attempt to recruit him or her. In the meantime you must choose an in-network dentist to have coverage under your plan.

Yes, family members can each choose a different PCD from the HDC Network. Just call 1-800-4-DENTAL (433-6825).
You can change your PDC as often as you like. Changes made prior to the 15th of the month will be effective on the first day of the following month. Call 1-800-4-DENTAL (433-6825) to change your PDC.
