Health Assessment
How healthy are you? Take your online health assessment.
Answer questions about your health history and lifestyle habits to learn more about your health, and get tips to help you stay well. You’ll also earn 100 NJWELL points toward your reward. Your covered spouse/partner can participate and earn points too.
Your individual results are kept completely confidential and are protected by federal and state privacy laws, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
You must have a HorizonBlue.com account to get started.
- Sign in to start your health assessment, follow the NJWELL prompts. Answer all questions and select Save and Continue on each page.
- At the end, select Finalize. Answer a few more questions, then select Finish to complete the health assessment.
- Your Health Score and the current date should appear. If the current date does not appear, your assessment was not completed, and your points will not be recorded. You may select Print Reports from the button menu to print a copy of your Summary Page and the Physician Summary Report to discuss with your doctor.
After you take the health assessment, select the Rewards tab to complete online activities or to enter the dates of preventive screenings to earn points toward your NJWELL reward.
How to earn rewards as a DPC member
If you are a Direct Primary Care (DPC) member, you can earn your reward by having a well visit and follow-up visit with your provider (700 points) and taking the health assessment (100 points). The well visit and follow-up visit need to take place between November 1, 2022 and October 31, 2023. You will earn the 700 points for the two visits once the claims are processed. The 100 points for the Health Assessment will be credit upon completion of the Health Assessment.
NJWELL, administered by the New Jersey Division of Pensions and Benefits, is an incentive-based wellness program offered to eligible employees and their covered spouses/partners who participate in the SHBP/SEHBP. All provisions of the program are established by the Division and are subject to change. More details can be found on nj.gov/njwell.
