Taking the health assessment and having a health screening are activities that everyone must complete to earn a reward.

If you are a Direct Primary Care (DPC) Member, you can earn 700 points with a well visit and follow-up visit. After you take the health assessment, share the report with your doctor. The well visit and follow-up visit need to take place between November 1, 2021 and October 31, 2022. You will earn 700 points once the claims are processed.


NJWELL, administered by the New Jersey Division of Pensions and Benefits, is an incentive-based wellness program offered to eligible employees and their covered spouses/partners who participate in the SHBP/SEHBP.
