Taking the health assessment and having a health screening are activities that everyone must complete to earn a reward.

If you are a Direct Primary Care (DPC) Member, you can earn 700 points with a well visit and follow-up visit. After you take the health assessment, share the report with your doctor. The well visit and follow-up visit need to take place between November 1, 2022 and October 31, 2023. You will earn 700 points once the claims are processed.
