Earn and Track Rewards
We want to make sure you understand and take advantage of your benefits. Eligible members and their covered spouses/partners can each earn Visa®Prepaid cards by getting preventive health screenings and participating in wellness activities.
You can receive points by learning about your health and taking steps toward improving your health. Simply complete an online health assessment and receive a health screening. You also have the opportunity to earn more points by participating in preventive health screenings, online activities and personal wellness coaching. Activities are listed on the NJWELL Rewards Lobby.
Each eligible SHBP or SEHBP member who completes an online health assessment, receives a health screening and earns at least 800 points by October 31, 2021 will receive a Visa® Prepaid card worth $250 or more. Visit nj.gov/njwell to learn more.
Here’s how you can earn a $250 Visa card¹ with NJWELL points.
- Take—or retake—the required online health assessment (100 points).
- Complete a health screening (100 points).
- 3. Complete activities of your choice or enter preventive screening dates. Earn additional points through October 31, 2021, by selecting the Rewards tab on the My Health Manager home page. Point values for activities vary and are posted on the NJWELL Reward Lobby page.
When you reach 800 points, you will receive an email with instructions to get your Visa card.² That’s a possible $250 in Visa cards¹ for each eligible member and covered spouse/partner—all for Taking Action and Working for a Healthy New Jersey! For more information, visit nj.gov/njwell.

Each eligible SHBP or SEHBP member who completes an online health assessment, receives a biometric health screening and earns at least 800 points by October 31 will receive a $250 Visa® Prepaid card. SEHBP non-Medicare retirees and their covered spouses/partners enrolled in NJ DIRECT Zero will earn $500, issued in two $250 prepaid cards, released within 24 days of earning the reward.

NJWELL is administered by Horizon BCBSNJ, your SHBP or SEHBP health plan. NJWELL Reward points are tracked and viewable through My Health Manager, powered by WebMD®:. When you sign in you can assess, track and manage your health care information from one location.
Yes. Your covered spouse/partner will need to register. This gives each of you secure access to your own personal information and the NJWELL reward activities, and the ability to track your own progress toward your financial reward.
To check your rewards points and status, visit the NJWELL Rewards Lobby. Simply sign in, select Wellness and scroll to NJWELL under My Health Manager. Select Learn more.
From the My Health Manager home page, choose NJWELL Rewards to view your NJWELL Rewards Lobby.
The health assessment registers as Completed in the NJWELL Rewards Lobby once you’ve responded to all questions (leaving no required fields blank) and received a personal health score between November 1, 2020 and October 31, 2021. If you did not receive a score, the health assessment was not completed. Remember, you must take the health assessment each program year.
Your health screening will be reflected as Completed once your results are submitted to, and successfully processed by, Quest Diagnostics (Quest). This process may take up to 10 days based on the method you select. You will be notified by Quest regarding your results status. Always remember to check your email for important messages regarding the status of your NJWELL participation.
Yes. You are rewarded for completed activities designed to promote healthy behaviors. The incentivized activities are designed to help you identify potential health risks and take ownership of your health.
