We want to make sure you understand and take advantage of your benefits. Eligible members and their covered spouses/partners can each earn Visa®Prepaid cards by getting preventive health screenings and participating in wellness activities.

You can receive points by learning about your health and taking steps toward improving your health. Simply complete an online health assessment and receive a health screening. You also have the opportunity to earn more points by participating in preventive health screenings, online activities and personal wellness coaching.

Each eligible SHBP or SEHBP member who completes an online health assessment and a health screening, and earns at least 800 points by October 31, 2021 will receive a Visa® Prepaid card worth $250 or more. The value of the reward is based on your plan. Visit nj.gov/njwell to learn more.

Steps to earn your reward

Here’s how you can earn a Visa card¹ with NJWELL points.

  1. Take–or retake–the required online health assessment (100 points). Sign in, select Wellness & Services and follow the NJWELL prompts.
  2. Visit my.questforhealth.com to choose how to complete a health screening. Use NJWELL as the registration key. (100 points). You’ll also earn more points if your test results are in normal range.
  3. Earn additional points by completing online activities and preventive screenings. To learn more, visit the NJWELL Rewards Lobby. Sign in, select Wellness & Services and follow the NJWELL prompts.
  4. 4. Once you’ve completed the health assessment, health screening and earned 800 points, WebMD® will notify you that you have earned your reward. Within 10 business days of earning the reward, you will receive an email from notification@prepaiddigitalsolutions.com with the subject line, Your NJWELL Visa Prepaid card is waiting for you to Redeem with instructions on how to access your Visa card.
