Retiree Wellness Program
We understand that good health is important to you and we appreciate that you are choosing to engage in healthy living.
The SHBP Retiree Wellness Program, administered by the state of New Jersey, helps eligible retirees get and stay healthy by understanding their health risks and getting yearly physical exams.
The program is voluntary, but your retirement health benefit contribution will be waived if you participate in the program. If you choose not to participate, a health contribution will be deducted from your retirement check.
Each year, eligible retirees who are not currently enrolled in the SHBP Retiree Wellness Program will be given the opportunity to enroll in the program, effective the following January.
Eligibility for the program is determined by the state of New Jersey.
If you are newly eligible, you will receive an invitation letter with instructions for enrollment and annual compliance. If your enrollment in the SHBP Retiree Wellness Program was terminated or you did not enroll when initially invited, you may enroll in the SHBP Retiree Wellness Program by completing an online health assessment between January 1 and October 31 of the current year. Your enrollment will be effective the following January.
To stay enrolled in the program,
- A Physician Certification form, confirming the retiree had a complete annual physical exam, must be completed and received by Horizon BCBSNJ each year by December 31.
- An online health assessment must be completed by the retiree each year by December 31.
- You must participate in the Chronic Care Program or Care Management Program, if identified.
- Take your Health Assessment.
To maintain enrollment in the SHBP Retiree Wellness Program, you must have a complete physical exam each calendar year you are enrolled in the program and submit an Annual Physician Certification each year to be received by Horizon BCBSNJ by December 31st.
