Retiree Wellness Program


We understand that good health is important to you and we appreciate that you are choosing to engage in healthy living.

The SHBP Retiree Wellness Program, administered by the state of New Jersey, helps eligible retirees get and stay healthy by understanding their health risks and getting yearly physical exams.

The program is voluntary, but your retirement health benefit contribution will be waived if you participate in the program. If you choose not to participate, a health contribution will be deducted from your retirement check.

Each year, eligible retirees who are not currently enrolled in the SHBP Retiree Wellness Program will be given the opportunity to enroll in the program, effective the following January.

Eligibility for the program is determined by the state of New Jersey.

If you are newly eligible, you will receive an invitation letter with instructions for enrollment and annual compliance. If your enrollment in the SHBP Retiree Wellness Program was terminated or you did not enroll when initially invited, you may enroll in the SHBP Retiree Wellness Program by completing an online health assessment between January 1 and October 31 of the current year. Your enrollment will be effective the following January.

State Health Benefits Program Retiree Wellness Program Annual Physician Certification

To maintain enrollment in the SHBP Retiree Wellness Program, you must have a complete physical exam each calendar year you are enrolled in the program and submit an Annual Physician Certification each year to be received by Horizon BCBSNJ by December 31st.
