Lost your member ID card? Display it on your device, print a replacement one and request a replacement. If you request a replacement, you will receive a new card in approximately five to 10 business days. Some members may only be able to view and print a proof of coverage letter, which includes the same information as a member ID card.

- Visit our Treatment Cost Estimator and Physician Review Tool to help you make better informed decisions about your health care.
- Access Horizon CareOnline℠ to talk with a licensed, board-certified doctor via video chat or phone.
- Send us a secure email. Once your question is submitted, you will receive a response from a Horizon BCBSNJ Member Services Representative within two business days. When the response is complete and available, an email will be sent to the address you are registered with, advising you to sign in. You must sign in to view the response.
