Horizon Supportive Care Program

Through this program, we work with CareCentrix, Inc. (CareCentrix) to manage member access to care in the post-acute care setting and to provide support to our members requiring palliative care.

CareCentrix provides utilization management and member support in the post-acute care setting as well as a community-based palliative care program for your patients enrolled in:

  • Horizon Commercial fully insured plans
  • Horizon Medicare Advantage plans
  • Braven Health℠ Medicare Advantage plans

Submit Authorization Requests for Post-Acute Care to CareCentrix

Post-acute facilities and hospitals will need to work directly with CareCentrix to request and receive authorization for eligible members, who are admitted to (or require extensions of their stay at) skilled nursing facilities (SNFs), subacute rehabilitation facilities (SARs), Transitional Care Units (TCUs) and inpatient rehabilitation facilities (IRFs).

  • Hospitals
    Hospitals may submit requests for post-acute facility authorizations to CareCentrix by phone or fax to the appropriate number below.

    Please note that hospitals do not currently have the ability to submit initial post-acute facility authorizations via the CareCentrix HomeBridge® portal. We will notify hospitals when this capability is available.

  • Post-acute Facilities
    Post-Acute Facilities can submit initial authorization requests through the CareCentrix HomeBridge portal or by phone or fax at the appropriate number below. Post-Acute Facilities can only submit continuation-of-services requests by phone or fax.
