Provider Data Maintenance Tool

Access this easy-to-use tool through NaviNet to update the provider demographic information, we have on file for you, including:

  • Add or delete office locations
  • Add or update office email addresses
  • Update phone and fax numbers
  • Manage practice limitations
  • Manage areas of expertise for behavioral health services
  • Indicate if you offer telemedicine services
  • Provide information about your race/ethnicity


Accessing the Provider Data Maintenance Tool

Your NaviNet Security Officer can use and provide you with access to our Provider Data Maintenance Tool. Talk to your NaviNet Security Officer today.

To identify your NaviNet Security Officer:

  • In the upper-right corner of any NaviNet screen, click My Profile and then click Manage My Profile.
  • Click My Security to view the Security Officer(s) within your organization.

Please Note

The following practice-level and practitioner changes cannot be submitted using our Provider Data Maintenance Tool and need to be submitted using the Horizon Data Submission Template for Practitioners.
  • Network terminations
  • TIN changes
  • Practice name changes
  • Practitioner specialty changes
