UB-04 Submission Capability to Sunset on NaviNet

Beginning September 1, 2021, the UB-04 Claim Submission transaction will no longer be available on NaviNet’s Horizon BCBSNJ plan central page.

Facilities may continue to submit UB-04 claim information electronically via Electronic Data Interchange (EDI) either directly, or through an authorized vendor/clearinghouse.

Electronic Claim Submissions

Electronic claims submissions help speed our reimbursement to you. We require all hospitals and ancillary providers to submit claims to us electronically. Visit our Electronic Data Interchange (EDI) webpages for details about submitting claims for our members.

Our EDI Service Desk is available to discuss your electronic claim submission options as well as enhancing your current practice management system with specifications for electronic submissions to us. Paper Submissions

If paper claims are necessary, we require that all paper claim submissions are printed on original, government-approved UB-04 claim forms and that you follow our Institutional Claim Submission Instructions.

Paper claims may be mailed to one of the following addresses.

BlueCard® claims:
