Valuable programs and services for members with heart conditions

According to the American Heart Association, nearly half of all U.S. adults have some form of cardiovascular disease¹. While this includes high blood pressure and cholesterol, it also includes heart failure. That’s why Horizon Blue Cross Blue Shield of New Jersey has a number of high-quality Care Management programs and services for your Horizon BCBSNJ patients with heart conditions.

Case Management

If a patient has a serious heart condition or needs heart surgery, our team of trained and fully licensed health care professionals and registered nurses, called Care Managers, can help patients better understand the diagnosis and follow treatment plans.

Case Management is a free and voluntary program available to all eligible Horizon BCBSNJ members.

Organ Transplant Program

Our Organ Transplant Program is available to eligible Horizon BCBSNJ through the Specialty Case Management Program. This program provides education, support and services patients may need throughout the transplant process. This is a voluntary program available to all Horizon BCBSNJ members at no additional cost.

Heart Failure Chronic Care Program

Our Chronic Care Programs help patients with chronic conditions better manage their health and improve their quality of life. The Heart Failure Chronic Care Program is offered at no cost to eligible members age 18 years and older who have been diagnosed with heart failure.

Cardiac Episode of Care Program

In an Episode of Care (EOC), the focus is on coordinated, collaborative care across the health care continuum to ensure that patients receive the highest quality and most comprehensive care. Using outcome measures developed in conjunction with our physician partners, our goals are to achieve the best long-term outcomes, create experiences that patients feel good about and reduce the total cost of care by eliminating unnecessary care.

The Cardiac Episode of Care Program is for eligible members with heart failure or Coronary Artery Disease (CAD), and for patients who need coronary artery bypass grafting.

Horizon BCBSNJ works with AbleTo, Inc.’s team of health care professionals to offer support by phone or secure live video.

Horizon BCBSNJ continually reviews medical information to identify members who may benefit from AbleTo. Members who are eligible for this program will receive a letter from Horizon BCBSNJ and/or a call from AbleTo to set up an initial consultation.
