HealthSphere — Patient data at your fingertips

HealthSphere is a health care data management platform that gives you a 360-degree view of your Horizon BCBSNJ patients’ medical records in real time. HealthSphere merges and organizes patients’ clinical information from various internal and external sources, so you can make sure your patients are getting the right care.

HorizonDocs — Document exchange and storage made easy

With HorizonDocs, we’ve made it easier for you to send us documents that we request and access documents we send, all in one place. HorizonDocs provides several additional benefits, including:

  • Provides a secure way to exchange documents and information.
  • Organizes documents by category and sub-category.
  • Allows you to control access to documents based on sensitivity level.
  • Sends email notifications when documents are sent and received.

Some examples of documents you may receive on HorizonDocs are lists of members who require screenings, Results & Recognition Reports, HEDIS® chart requests, and Electronic Health Records (EHR) Data Submission Templates.
