Code Terminations as the PHE Ends

The Biden Administration will end the COVID-19 public health emergency (PHE) on May 11, 2023.

Certain changes we had put in place during the PHE will go back to pre-COVID-19 coverage on May 12, 2023. In addition, certain codes will no longer be accepted.

COVID-19 Testing

Horizon will not consider the SARS-CoV-2 testing services represented by the HCPCS codes below for reimbursement when testing is performed after May 11, 2023.

COVID Vaccinations

Per the AMA's CPT Editorial Panel, effective April 18, 2023, the monovalent Moderna and Pfizer-BioNTech COVID-19 vaccines (represented by the codes below) are no longer authorized for use in the United States.

Horizon will not consider these codes for reimbursement for services rendered on and after April 18, 2023.
