Exercise regularly? Get rewarded! HorizonbFit is a fitness incentive program that rewards you for making your health and fitness a priority.
HorizonbFit℠ rewards eligible members for getting regular exercise. Once you enroll, you become eligible to receive a $20 reward for every month in which you:
- Visit one of 30,000 accessible participating fitness facilities 12 or more days a month
- Walk 10,000 steps a day for at least 12 days a month, or
- Complete any combination of visiting a fitness facility and/or walking 10,000 steps for a total of at least 12 days a month (e.g., within the month, you visit a participating gym for six days and walk 10,000 steps for six days).
With HorizonbFit, you can earn up to $240 a year in rewards! We want to help our members get and stay healthy.
HorizonbFit uses the free ActiveFit™ mobile app to make syncing your step count and reporting gym visits easy. ActiveFit imports your steps from a compatible health app (Apple Health, Google Fit™ and Fitbit®). It also uses your phone’s GPS and Bluetooth services to detect when you’ve entered your gym and records and reports your activity for your monthly rewards.
Whether you’re walking or hitting the gym, ActiveFit does the tracking for you. Download it free from the App Store℠ or Google Play™.
How to enroll
Enrolling is free and easy at HorizonbFit.com. Here you can verify your eligibility using your member ID card number and set up your account. If your fitness facility does not participate with HorizonbFit, you can nominate it for inclusion.

No. If you are using ActiveFit, your attendance information is directly transmitted to your account. You will be able to see your last visit, total visits, payment information and more on your smartphone. If you are not using ActiveFit, you will be able to view the attendance submitted by your fitness facility after the facility submits its monthly report.
If you have met the attendance goal, your incentive will be sent to you via your linked debit or credit card within 60 days following the month in which it was earned. You will receive an email when you have reached your attendance goal and another when your incentives payment has been sent.
Eligibility for the program is based on you remaining enrolled in an eligible health plan, and on offering the fitness reimbursement program. If at any time we cancel HorizonbFit, you will be notified and you will not be eligible for any future rewards. If you were actively covered by us in the month(s) in which you qualified for rewards, your account will be fully credited for the incentives earned.
Research shows that regular exercise can help you reduce your risk for several diseases and health conditions, maintain a healthy weight and improve your quality of life. HorizonbFit can help you realize these benefits by providing incentives that make it easier and more affordable to exercise regularly.
You can visit HorizonbFit.com to verify your eligibility. Your benefits administrator or human resources department also will be able to confirm if your health plan participates in HorizonbFit and if you are eligible.
Attendance data is tracked in two ways:
Through ActiveFit™, a smartphone app that can be downloaded from HorizonbFit.com during the enrollment process. ActiveFit:
- Provides access to visit and payment information
- Has a step counter and can share motivational messaging to keep your workouts on track
- Can enable you to nominate a fitness facility into the network
Through attendance records uploaded from your in-network facility to Advanta Health Solutions, which administers the program. Visits are viewable at the end of the month in your personal member portal. All providers displayed on the HorizonbFit map can both upload facility-generated attendance reports and support ActiveFit mobile tracking.
If you are eligible for HorizonbFit, your family members who are 18 or older and covered under your health plan are also eligible to participate in the program.
If you are currently a member at a participating fitness facility, then you just need to complete the enrollment form at HorizonbFit.com, create your HorizonbFit account on the secure site and link it to a major credit or debit card. Your existing membership privileges will remain unchanged, and you will continue to pay your membership fees directly to your fitness facility. Your linked credit or debit card is only used by HorizonbFit to process your reimbursements.
No. Member enrollment in HorizonbFit is free. However, you must be a member of an in-network fitness facility and be current with your membership dues to be eligible for incentives.
