If your plan uses the Horizon Managed Care Network (Horizon HMO, Horizon Advantage EPO, OMNIA℠ Health Plans, Horizon Direct Access®, Horizon POS, Horizon Medicare Advantage or NJ DIRECT and Value Access plans), you have in-network access to the following laboratories:

If you are enrolled in a Horizon managed care plan, pathology services provided in a hospital setting, by a practice that participates in the Horizon Managed Care Network, are allowed. This allowance is an exception to the above-described LabCorp/Quest Diagnostics network use requirements.

We work with eviCore Healthcare for non-emergency outpatient radiology and diagnostic imaging services.
If your PCP decides that you need an X-ray or scan, call eviCore Healthcare's Scheduling Line at 1-866-969-1234, weekdays, from 7 a.m. to 7 p.m., ET.
An eviCore Healthcare representative will:
- Find an in-network facility near you.
- Coordinate with the in-network imaging center of your choice to schedule your exam and give you a confirmation number. You don't need a referral for most radiology services.
- Decide whether a service is medically necessary.

Your doctor must call eviCore Healthcare before you get these services:
- CT/CTA scans
- Diagnostic left-heart catheterization
- Echo stress tests
- Echocardiograms
- MRIs/MRAs
- Nuclear medicine studies (including nuclear cardiology)
- PET scans
You can visit one of our many in-network hospitals, labs, imaging centers and other facilities. They offer quality care while keeping your out-of-pocket costs low. To find an in-network hospital or facility near you, use our Doctor & Hospital Finder or the Horizon Blue app.
Urgent Care Centers
When you have a non-life-threatening injury or illness and can't see your doctor, an Urgent Care Center can often treat you faster with lower out-of-pocket costs than an Emergency Room (ER).
With evening and weekend hours, Urgent Care Centers treat wounds, sprains and other health issues that need quick care.
You do not need a referral to go to a participating Urgent Care Center, but you must pay your copayment, coinsurance and/or deductible.
Please note: At participating Urgent Care Centers, routine office visits, including preventive care, sports physicals, routine obstetric services, occupational medicine and physical therapy services are not covered.

You can get quality laboratory services as ordered by your doctor and based on your plan benefits. You pay less when you use in-network preferred laboratories.
