Update to the Horizon Care@Home Program
The Horizon Care@Home Program, currently administered by CareCentrix, will begin to transition the management of certain home health services (including in-home nursing services and physical, occupational and speech therapy) back to Horizon Blue Cross Blue Shield of New Jersey.
This change is effective November 1, 2016. Through the Horizon Care@Home Program, CareCentrix will continue to coordinate and manage:
- Durable medical equipment (DME), which also includes diabetic supplies, medical foods and home medical supplies
- Orthotics and prosthetics (O&P)
- Home infusion therapy (HIT) services
- Hemophilia factor drugs
During the transition, Horizon BCBSNJ and CareCentrix will focus on ensuring you continue to have access to all home health care services that are medically necessary. You should continue to access all home health care services through the Horizon Care@Home Program, as your benefits will not change or be impacted.
