Getting proof of health coverage

If you need medical care before you receive your member ID card, you can get proof of coverage through one of the following options:

  • Sign in to Member Online Services. Then, click Eligibility Details from the My Plan section. Your 2016 enrollment status, the Horizon BCBSNJ product you chose, your Horizon BCBSNJ member ID number and covered dependents are listed in this section.
  • Request a Certificate of Creditable Coverage (COCC) Letter from a Member Services Representative at 1-800-355-BLUE (2583), Monday through Wednesday and Friday, from 8 a.m. to 6 p.m. Eastern Time (ET), and Thursday, from 9 a.m. to 6 p.m., ET. Member Services Representatives can mail, email or fax you a COCC Letter. Member Services Representatives can also call your doctor’s office to verify your coverage.

Your participating doctor can also access your eligibility information.
