The Horizon Supportive Care Program℠ is a free, voluntary program that can connect you to palliative care services, when you need them.
What is palliative care?
Palliative care is specialized medical care for people living with serious illnesses. This type of care focuses on relief of the symptoms and stress from the illness. The goal is to improve the quality of life for the patient and the family.
Supportive Care Program: Helping Members at a Time of Need
How we can help
We’ll work with you, your family members and your doctor to:
- Identify your health goals. You’ll get help with treatment planning, and emotional and spiritual support, as well as coordination of care and access to community services.
- Help you understand your benefits and coverage. Horizon Supportive Care Program nurses will work together with your doctor and other health care professionals to help manage pain and other physical or emotional distress.
- Address your specific needs and ensure you get the care that’s right for you.
