Find participating doctors and hospitals in and outside of New Jersey

As a Horizon BCBSNJ member, you have access to a broad network of quality participating doctors, health care professionals and hospitals.

Depending on your plan, you may choose from:

  • More than 41,000¹ doctors and other health care professionals, including specialty and behavioral health care professionals
  • 82¹ hospitals in 100 locations in New Jersey, Pennsylvania and Delaware¹

Horizon BCBSNJ has the tools and resources you need to help manage your care. We offer easy access to online information to help you find an in-network doctor based on your plan, compare hospitals based on specific diagnoses and procedures, and more.

Select a Primary Care Physician (PCP)

Your plan may require you to select a PCP. Your PCP is responsible for helping you coordinate your care, and can identify and treat common medical conditions, as well as provide preventive care and counseling on healthy lifestyle choices. When choosing a PCP, consider whether you want a doctor who is board certified, and has special training or admitting privileges at the hospital of your choice.

Living or traveling outside of New Jersey?

Members enrolled in plans that include BlueCard® benefits have access to participating doctors and hospitals throughout the country. Ninety-two percent of doctors and 96 percent of hospitals in the United States participate in the BlueCard Program.²

To find doctors and hospitals available to you through the BlueCard Program:

  1. Have your Horizon BCBSNJ ID card on hand.
  2. Visit and select Doctors Outside of NJ within the What are you looking for? dropdown menu.
  3. Enter the required information and click Search.

Or you can call BlueCard Access at 1-800-810-BLUE (2583) for the names and addresses of doctors and hospitals in the area where you or your covered dependent need care.
