Appeal Responses Now Faxed


Effective immediately, Horizon Casualty Services will fax responses to appeal requests. This will enable HCS to deliver responses more efficiently.

To ensure a timely response to your appeal, please provide a fax number when submitting the appeal, and include the reason for the appeal, a copy of the bill and the Explanation of Payment (EOP) Statement.
