  • If you purchased coverage through the NJ State Based Exchange (SBE): Go to Get Covered New Jersey or call 1-800-224-1234.

    If you bought insurance directly from Horizon BCBSNJ: You must complete and return the Non-Group Enrollment/Change Request Form to change your coverage information, including adding or removing an eligible spouse or dependent, changing your name, changing your plan, and other Special Enrollment Period (SEP) related changes. You must submit the application and supporting documentation, if applicable¹, within 60 days from the date of the loss of coverage or life event.

    ¹ During the Special Enrollment Period (SEP) for COVID-19, you may change your coverage without providing supporting documentation.

  • For members with coverage through an employer: Contact your employer’s benefits administrator or human resources department to cancel your coverage.

    If you purchased your coverage through the Exchange (the federal Marketplace): Go to HealthCare.gov or call the Exchange Call Center at 1-800-318-2596 to cancel your coverage. Please provide the Exchange at least 14 days advance notice of when you want to terminate your coverage.

    If you purchased your health coverage through the NJ State Based Exchange (SBE): Go to Get Covered New Jersey or call 1-800-224-1234 to cancel your coverage. Please provide the SBE at least 14 days advance notice of when you want to terminate your coverage.

    If you purchased your coverage directly through Horizon BCBSNJ and not through the Exchange (the federal Marketplace) or the NJ State Based Exchange (SBE): You can voluntarily terminate your coverage at any time for any reason with a future termination date. Complete the Request for Termination Form and submit it using one of the options on the form.

  • For members with coverage through an employer: If you have coverage through an employer, please contact your employer’s benefits administrator or human resources department directly.

    For members with coverage NOT through an employer: You will need to provide a copy of your dependent child’s birth certificate or driver’s license to show proof of age to add or terminate a dependent from your coverage during a special enrollment period¹. During Annual Open Enrollment, you are not required to show proof of a dependent’s age.

    ¹ You can make changes to your coverage if you experience loss of coverage or certain qualifying life events, which allows you to enroll in health insurance outside the yearly Open Enrollment Period, also known as a Special Enrollment Period (SEP). If you make changes to your coverage during this timeframe, you are required to provide proof of the life event and the date it occurred. In addition, beginning in 2021, the federal government determined that COVID-19 caused significant challenges for consumers in getting health insurance and declared a SEP from February 1 to August 15, 2021. If you add or remove a dependent from your coverage during the COVID-19 SEP, no documentation is required.

  • In 2021, the federal government determined that COVID-19 caused significant challenges for consumers in getting health insurance and declared a Special Open Enrollment Period (SEP) from February 1 to August 15, 2021.

    The new COVID-19 SEP allows:

    • Uninsured individuals to enroll in an individual plan;
    • Existing members to change from one individual plan to another;
    • Existing group members to change to an individual plan; and
    • Existing members to add or remove a dependent from their individual policy.

    The COVID-19 SEP operates like the Annual Open Enrollment Period, since there is no triggering life event and no supporting documentation required.

    I get my insurance through work:

    If you get your coverage through your employer, check with your benefits administrator or human resources department for information about your employer’s Open Enrollment Period. Normally, you can only make changes to your coverage outside of the Open Enrollment Period when you have a qualifying life event. If you are an SHBP/SEHBP retiree, call or email the Division of Pensions and Benefits or, visit state.nj.us/treasury/pensions/fact-sheets.shtml.

    I bought my coverage from the NJ State Based Exchange (SBE) or from Horizon BCBSNJ directly:

    You can change your coverage:

    1. During the Open Enrollment Period – November 1 through January 31. You do not need to provide proof of qualifying life event changes.
    2. During the Special Enrollment Period (SEP) for COVID-19 – February 1 to August 15, 2021. You do not need to provide proof of life event changes.
    3. If you experience a qualifying life event – at any time. If you change coverage during this timeframe, you are required to provide proof of the life event and the date it occurred.
