How can I make changes to my coverage?

If you purchased coverage through the NJ State Based Exchange (SBE): Go to Get Covered New Jersey or call 1-800-224-1234.

If you bought insurance directly from Horizon BCBSNJ: You must complete and return the Non-Group Enrollment/Change Request Form to change your coverage information, including adding or removing an eligible spouse or dependent, changing your name, changing your plan, and other Special Enrollment Period (SEP) related changes. You must submit the application and supporting documentation, if applicable¹, within 60 days from the date of the loss of coverage or life event.
