  • Yes, you still have access to care. As a member, you do not need to have a physical member ID card to get care from your doctor or other health care professional. You can:

    • If it’s available online, sign in to print a copy of your member ID card or proof of coverage letter and bring it with you. Or, use the Horizon Blue app to download a copy of your member ID card or proof of coverage letter on your smartphone. To download the Horizon Blue app, text GetApp to 422-272.
    • If your member ID card or proof of coverage letter is not available online, you can request a proof of coverage letter by mail when you complete the request form, or when you use our Email Us or Chat tools.
    • Have your doctor, facility or other health care professional look up your eligibility and member ID card from our online provider tools.

  • Medical Member ID Cards

    You can sign in to print your medical member ID card or use the Horizon Blue app within 48 to 72 hours after your enrollment has been processed. Your member ID card will be mailed within 5-10 business days after you are enrolled in your selected plan. Member ID cards are not mailed every year, but are typically mailed when you change plans, have a change in level of coverage, or if you are enrolled in an HMO plan and change your Primary Care Physician (PCP). Other situations may also apply.

    If you have a new plan and your member ID card is not available when you try to view it online, your enrollment request may still be processing. If you have questions about the status of your enrollment, use our Email Us or Chat tools and select Enrollment.

    Dental Member ID Cards

    You can sign in to view your dental ID card online or use the Horizon Blue app, unless you’re enrolled in Horizon Dental Choice. If you’re enrolled in Horizon Dental Choice, call Horizon Dental at 1-800-4DENTAL (433-6825) for additional information about dental ID cards.

    Vision Member ID Cards

    If you have a Davis Vision plan, visit davisvision.com and register for an account to view your vision plan details. For more information regarding your vision member ID card, call Davis Vision at 1-800-278-7753.

  • There is a lot of important information on your member ID card. Here’s what it all means.

    Sample member ID card
    1. Member Name: In most cases, this will only show the subscriber’s name, not the covered dependent’s name. The doctor and hospital will accept this card for you and your covered dependents.
    2. Member ID Number: Your member ID number is made up of a series of letters and numbers. It will be the same for everyone covered under your Horizon plan and will remain as your ID number for as long as you are the subscriber of the health plan. Generally, the numbers after the 3HZN are what doctors and hospitals will need from you. The prefix (the first three letters/numbers before the 3HZN) is used by Horizon to route claims for processing.
    3. Coverage Verification Data: Doctors and hospitals need this information to check eligibility and submit claims. They can also sign in to the provider website to verify your coverage.
    4. Primary Care Physician (PCP) name, if a PCP is required by your plan.
    5. Name of your Horizon Plan: You will need to know this when looking for participating doctors and hospitals in the Doctor & Hospital Finder.
    6. Copayment Information: This will show only if your plan includes a copayment. The copayment is how much you pay, if applicable, each time you get care, treatment or services. The amount may vary depending on where you receive care (i.e., hospital, primary doctor or specialist). Your copayment is not your coinsurance amount. Some member ID cards will also show your coinsurance and deductible amount, if this applies to your plan.
    7. Suitcase Logo: If you have the “PPO in a suitcase” picture, it means you can get in-network care when traveling outside New Jersey from a doctor or hospital who participates with the local Blue Cross and/or Blue Shield Plan. If you have an empty suitcase logo, nonemergency care received outside of New Jersey will be covered as out-of-network.
    8. Member and provider claim filing information.
    9. Indication of whether your Horizon Plan is Insured or Self-funded: This has to do with how your employer, if applicable, administers your Horizon health plan.
    10. Website Address: Go online for information about claims, benefits, and the exclusive tools and resources available to you. Remember to register to access these tools and resources.
    11. Important Phone Numbers for you and your doctors.
    12. Logo indicating you have prescription drug coverage through your Horizon health plan.

    If you have not received your member ID card yet or you lost it, don’t worry. Simply sign in to view and print an image of your card, or request a new one. You can also text “GetApp” to 42272 to download the HorizonBlue app to view and share your member ID card. Doctors and hospitals will accept this image of your ID card. Your doctor and hospital staff can also look up your ID card, eligibility and benefits online.

  • If your policy is currently active, you can view, print, download or request a new member ID card. If you are enrolled in more than one Horizon plan, select the card you need from the menu. Click New Card to request to have additional member ID cards mailed to you.

  • If your policy is currently active but no member ID card is loaded online, your enrollment may still be processing. In this case, you can view, print or download a proof of coverage letter.

    If you are not able to view, print or download a member ID card or proof of coverage letter, your policy may not be active. Use our Email Us or Chat tools and select Enrollment to contact us with any questions.
