For members with coverage through an employer: Contact your employer’s benefits administrator or human resources department to cancel your coverage.
If you purchased your health coverage through the NJ state-based exchange (SBE): Go to Get Covered New Jersey or call 1-833-677-1010 (TTY 711). Please provide the SBE at least 14 days advance notice of when you want to terminate your coverage.
If you purchased your coverage directly through Horizon BCBSNJ and not through the NJ state-based exchange (SBE): You can voluntarily terminate your coverage at any time for any reason with a future termination date. Complete the Request for Termination Form and submit it using one of the options on the form.

You cannot update your address using our member website or the Horizon Blue App. Instead, to request that we change the address we have on file for you, be sure to provide your name, member ID number and new address when contacting us below.
If you purchased coverage through the NJ state-based exchange (SBE): Go to Get Covered New Jersey or call 1-833-677-1010 (TTY 711).
If you purchased your insurance directly through Horizon:
- Use our Email Us tool. Under Category, choose Enrollment;
- Fax your information to 1-973-274-4413; or
- Check the box titled Report Address Change and complete the information on your invoice when sending your premium payment by mail.
State Health Benefits Program (SHBP) and School Employees’ Health Benefits Program (SEHBP) members except for Rutgers employees:
Log into Benefitsolver, a tool of the SHBP/SEHBP via your MyNewJersey account. Rutgers employees must update their address and phone number directly with Rutgers.
Medicare Supplement members (other than SHBP and SEHBP members):
- Use our Email Us tool. Under Category, choose Enrollment; or
- Send a written request to: Horizon, PO Box 10138, Newark, NJ 07101
For other members with coverage through an employer not mentioned above: You may need to work directly with your employer’s benefits administrator or human resources department to update your mailing address. If you are not sure if this applies to you or if you need additional help, contact us through our Email Us or Chat tools. We will let you know if you need to contact your employer directly, or if we can make the change.

It depends on how you got your coverage and when you’re trying to make a change.
If you bought your insurance from Get Covered NJ (the state-based exchange) or directly from Horizon:
You can change your coverage:
- During the Open Enrollment Period: November 1 through January 31.
- If you experience a qualifying life event at any time during the year. You are required to provide proof of the life event and the changes to your coverage within 60 days after the event.
If you are an SHBP/SEHBP retiree:
Contact the Division of Pensions and Benefits.
If you get your insurance through work:
Check with your benefits administrator or human resources department. Normally, you can only make changes to your coverage during the Open Enrollment Period or when you have a qualifying life event.

Turning 26 years old is considered a qualifying life event, so your dependent has options for getting coverage. Which options they have will depend on which of the following situations apply to them:

- Your dependent is eligible to enroll in their employer’s plan at any time. They can contact Human Resources or other representative at their employer within 30 days of losing coverage to start this process.
- If your dependent decides not to enroll in their employer’s plan, they will likely NOT be eligible for a discount when buying a plan through GetCoveredNJ (the state-based health insurance exchange).

- Your dependent is eligible to buy health insurance through GetCoveredNJ during a Special Enrollment Period (SEP). The SEP will begin 60 days before they lose coverage and end 60 days after.
- If your dependent enrolls in a plan through GetCoveredNJ before they lose coverage, that plan can start the first day of the month after they lose coverage.
- If your dependent enrolls in a plan through GetCoveredNJ after they lose coverage, the new plan can start the first day of the month after they sign up.
- Your dependent is eligible to buy health insurance through GetCoveredNJ during a Special Enrollment Period (SEP). The SEP will begin 60 days before they lose coverage and end 60 days after.

- After your dependent turns 26 years old, but before they turn 31 years old, they may be eligible to enroll as a dependent in a parent’s job-based plan by making a “DU31 election.” If your dependent already has their own coverage (other than Medicare) at that time, they would have to give up their existing policy to enroll in your health plan. The DU31 law does not apply if you have an individual or family health plan purchased through GetCoveredNJ (the state-based health insurance exchange).

They may be eligible for extended coverage under New Jersey’s Small Group Continuation (NJSGC) law. Speak with your employer for more information on how to request the extension.

They may be eligible to buy an extension of health insurance coverage for up to 36 months, under Consolidated Omnibus Budget Reconciliation Act (COBRA). To sign up for COBRA coverage, notify your employer in writing within 60 days of your dependent turning 26 years old (if the plan doesn’t notify you of this option first).

If your dependent has coverage under your health insurance plan, their dental and vision benefits will end on the date of their 26th birthday. However, you have three options to potentially extend their coverage. You can apply for COBRA continuation coverage, use New Jersey’s Small Group Continuation (NJSGC) law or choose to apply for the New Jersey Dependent Under 31 (DU31) election.
