Did you know your EOB is available online, days before you’d get it in the mail? Now with digital EOBs, you get the claim details you need faster and more securely.
An EOB, or Explanation of Benefits statement, gives you important information about your claims, including:
- the amount your plan allows for the treatment, service or device
- how much we’ve paid
- what — if anything — you owe.
Already registered for our portal or the Horizon Blue app? You’re all set.
When your claim is processed, you’ll get an email or SMS text — depending on your preference — letting you know that an EOB is ready for your review.
To view, download or print EOBs from the past 18 months, sign in or open the Horizon Blue app. Then:
- Click or Tap Claims.
- Locate the appropriate claim, and then click or tap the claim number.
- Click View Explanation of Benefits.
Not registered for our portal or the Horizon Blue app?
Once you register for our secure member web portal, you can download the Horizon Blue app. Need help registering? Call the eService Desk at 1-888-777-5075, weekdays, from 7 a.m. to 6 p.m., Eastern Time.
Need a paper EOB?
You can contact Member Services through email, chat or by phone to ask for a specific EOB to be mailed to you.
To go back to receiving all of your EOBs by mail, sign in to the secure member web portal or Horizon Blue app and go to Profile & Settings. Then go to Paperless Settings and uncheck the Go Paperless box. There is no fee for receiving paper EOBs.
If you are not registered on our secure member web portal, you will receive your EOB in the mail within 21 days of our receipt of a claim.
