Grantee Information
These resources will assist you during the grant period by providing guidance on publicly announcing your grant award, keeping the Horizon Foundation informed about your grant activities, and submitting your final report.
The Horizon Foundation for New Jersey looks forward to working with you throughout your grant period.

- You will receive an official award email that will include instructions on how to access your Grant Agreement and Bank Authorization Form.
- You may also access your grant documents by logging into your Cybergrants account.
- Grant funds are directly deposited into an approved bank account within 45 business days of receiving the completed grant documents.

- Grantees are responsible for submitting a comprehensive Final Report on the results of their project. Your grant period dates are stated in your grant agreement letter.
- If you submit a new grant application prior to the end of the grant term, then a final report must be submitted at the same time as the new grant application.
- When submitting your final report, provide detailed outcomes for the measures indicated on your application. For guidance, please review our measurement requirements.
- Final reports are accepted online only. Sign in to access the final report form.

- Upon receipt of your completed grant documents, you will receive a Media Kit to assist you in promoting your grant. The Media Kit includes the Foundation logo, a sample press release, social media guidance and a storytelling toolkit.
- Refer to your grant agreement for additional guidance on acknowledging the Foundation during the grant period.
- Acknowledgement activities and details will be required as part of your Final Report.

Grant renewals are not automatic and cannot be guaranteed from year to year. Before applying for a renewal grant, contact your Program Officer to inform them of your plans to apply.
Grantees may apply for a renewal grant before the current grant period has ended. However, you are responsible for submitting a comprehensive Final Report on the results of the project supported by The Horizon Foundation grant. The report must be submitted at the same time as the renewal grant application.

While every program is unique and may address different goals, having the ability to demonstrate the Foundation’s collective impact across our grants portfolio is important. Organizations that receive grant awards must track their data and report on the measurement indicators within their respective priority area.
Obesity
Percent of participants who reported:
- Decreased consumption of sugar, sweetened beverages and candy.
- Increased consumption of milk, fruit, vegetables and water regularly.
- Increased physical activity to a minimum of 30 minutes daily.
- Increased health education and nutritional knowledge.
- Number of participants who received a health screening. For example: A1C diabetes test, weight/BMI or other risk assessments.
If your program addresses healthy food accessibility, you must also report on the percent of participants who were referred to places and programs where they could access healthy foods.
If your program addresses public spaces, you must also report on the percent of residents who reported they used the public space and the percent of residents who reported they feel safe using the public space.
Opioid
Percent of participants who reported:
- Increased awareness of community resources to help someone access treatment for opioid use disorder.
- They have a better understanding of how prescribed opioid medications may lead to an addiction.
- They are aware that fentanyl is a prescribed synthetic opioid that is sometimes added to heroin and can lead to a higher risk for overdose and death.
- They learned where to safely dispose of expired, unused or unwanted prescription medication.
- Increased understanding of the stigma associated with opioid use disorder.
Mental Health
Percent of participants who reported:
- Increased awareness of community prevention and support resources to address mental health disorders.
- Increased understanding of the stigma associated with mental health disorders.
- They received information about social supports that helped increase coping and life skills for people with mental health disorders.
- They experienced an overall improvement in their quality of life.
Connecting - Social Determinants of Health
Percent of participants who reported:
- Improved quality of life due to the services provided.
- They received accessible referrals or services for social needs.
- Increased ability to better self-manage their health care and social needs.
- Improvement in their physical and mental health. For example: weight loss, increased physical activity, medication adherence, engagement with a life coach, completion of a mental health screen, etc.
- Decreased inappropriate/excess inpatient utilization, emergency room visits and/or hospital readmissions.
