Horizon Medicare Blue Access Group (HMO-POS)
With a Horizon Medicare Blue Access Group (HMO-POS) plan, members can save by using doctors and health care professionals who participate in the Horizon Managed Care Network – without referrals. In most cases, members can keep the same doctors, health care professionals and hospitals they used with their under-65 Horizon Group coverage. Options with both medical and prescription drug benefits are also available.
>Horizon Medicare Group HMO-POS Plans:
- Available to retirees who live in New Jersey.
- PCP selection is not required but recommended. If you do not select one, you may have to pay a Specialist copay when you see a general practitioner.
- Members can save by using doctors and health care professionals who participate in the Horizon Managed Care Network – without referrals.

>How HMO-POS coverage works:
- Horizon Medicare Blue Access Group (HMO-POS) is a Health Maintenance Organization (HMO) plan that encourages members to use in-network providers, but offers the flexibility to see out-of-network doctors, specialists and other health care professionals in New Jersey.
- PCP selection is not required but recommended. If you do not select one, you may have to pay a specialist copay when you see a general practitioner.
- The plan may have separate deductibles and out-of-pocket limits for in- and out-of-network services.
