Form 1095-B and Health Coverage Information

Beginning January 31, 2018, Horizon Blue Cross Blue Shield of New Jersey will mail Form 1095-B to your employees who are enrolled in Horizon BCBSNJ coverage through your employer-sponsored health plan. Your employees can also sign into Member Online Services at HorizonBlue.com to view their form. A sample of the letter to members, along with a sample of Form 1095-B, is attached.

Form 1095-B provides information about your employees’ Horizon BCBSNJ health insurance coverage to help them properly prepare their tax return for the Internal Revenue Service (IRS). Form 1095-B includes the following information for your employees and, if applicable, their covered dependents:

  • Name(s)
  • Last four digits of the Social Security Number(s) (SSN)
  • Date(s) of birth (if SSN not available)
  • Months of health insurance coverage with Horizon BCBSNJ from January 1, 2017 through December 31, 2017

Form 1095-B is informational and is issued to help your employees comply with their responsibilities under the Affordable Care Act (ACA); it does not need to be attached to their federal income tax return.

If your employees think their information is incorrect, please instruct them to call Member Services at the number on the back of their member ID card.

Horizon BCBSNJ will report to the IRS the months that your employees and their covered dependents had qualifying health coverage through Horizon BCBSNJ. Horizon BCBSNJ can only provide your employees with the months that they and their qualified dependents had fully insured Horizon BCBSNJ coverage. If, at any time during the year, your employee had coverage through a different employer or insurer, this employee will receive additional forms from that employer or insurer. If your employee bought their coverage through the Health Insurance Marketplace at any time in 2017, they will receive Form 1095-A from the Centers for Medicare & Medicaid Services (CMS).
