For easy, on-the-go access, use the Horizon Blue app for:

  • Instant access to coverage details, including your member ID card and claims.
  • Doctor video chat1 and nurse live chat.
  • Appointment scheduling.
  • Fast answers to coverage questions.

To get started:

  1. Register for HorizonBlue.com.
  2. Download the free Horizon Blue app by texting GetApp to 422-272 or visit the Apple® App Store® or Google Play.2
  3. Install the app on your device.
  4. Sign in using the username and password you chose for HorizonBlue.com.

Currently and through at least 90 days after the end of the COVID-19 public health emergency, you will not pay out of pocket when receiving care for covered services through Horizon CareOnline. Once this period is over, you will pay:

  • PPO Plan: $5 copay for urgent medical visits and behavioral health visits.
  • CDHP: Medical care — no more than $59 per visit. Behavioral health visits — range from $79 to $175, depending on the type of service and therapist.

For members in CDHP, you will be charged $5 on your credit or debit card. Once the claim is processed, you will be billed the outstanding balance. However, if you have met your out-of-pocket maximum, you will not be charged.


There’s no charge to download the Horizon Blue app, but rates from your wireless provider may apply.
