The Value of Horizon’s Health Plan

Your client’s Horizon health plan is about more than just paying medical bills. For nearly 90 years, we’ve been helping our members get the care they need, when they need it. That’s why we offer the convenience of telemedicine, programs to support our members and no-cost preventive care.

Convenient access to telemedicine
The first step to using telemedicine to get care is to register at HorizonBlue.com and the Horizon Blue app¹. Once registered, members can:

  • Get care for common health problems like a cold or the flu through Horizon CareOnline℠². U.S. board-certified, licensed doctors provide care by video or phone, 24/7, with no appointment needed. Eligible members can also make an appointment with a behavioral health professional.
  • Get answers to their health questions like information about health screenings and immunizations, at no cost, 24/7, from a registered nurse through Nurse Chat.

And, that’s not all. Members can sign in to HorizonBlue.com and theHorizon Blue app to send a copy of their member ID card to their doctor or covered dependents, follow the prompts to print or download their ID card, and review their claims information. When using the Horizon Blue app, members can also save their member ID card to a digital wallet on their phone for easy access.

Members can also use the online Doctor & Hospital Finder to locate in-network doctors, hospitals and other health care professionals.

Programs to support our members
Our Care Management Program is for eligible members facing a life-changing medical condition, like a cancer diagnosis, an organ transplant or a high-risk pregnancy. Our Care Managers can help members talk about their medical condition with their doctor, guide them to the best care possible and help them manage their overall health. Members can learn more by visiting HorizonBlue.com or by calling 1-888-621-5894, option 2.

With the new Horizon Behavioral Health website, members can access a wealth of resources with one click. The site makes it easier for members to get needed information about mental health conditions and Substance Use Disorders. Members can easily contact the Horizon Behavioral Health team for help connecting with care and getting necessary follow‐up support.

Preventive care at no additional cost
Preventive care is more than maintaining a healthy lifestyle, although exercising regularly, washing your hands, eating a healthy diet and getting enough sleep, all help. We want to help our members achieve their best health, which means getting the recommended immunizations and screenings to avoid getting sick. Because it’s so important, most preventive care services are covered 100 percent when in-network doctors provide the care.
