New Electronic Group Application Now Available

Applies to: 51+ groups and Public Sector market

Horizon is working hard to reduce the administrative process for brokers during the sales and implementation stages. Due to feedback about the paperwork, we’ve revised our Group Application for groups with 51 or more employees and the Public Sector market. Our new digital application is easier for you to complete and submit for group health and ancillary coverage.

The new digital Group Application replaces all previous versions of the application and should be used immediately. Paper versions of the application will no longer be accepted and may cause delays in processing your group customers’ requests.
