Member ID Cards and Proof of Coverage Options Available Through App and Online

Members who need care but don't have their member ID handy can easily use the Horizon Blue app to access their card and show it to a health care professional.
Using the app, members can display and/or print a copy of their member ID card – as well as order a permanent replacement. Members just need to register for the app, sign in, choose ID Cards and follow the instructions. To download the Horizon Blue app, members can text GetApp to 422-272 or download it at the App Store® or Google Play™.
Members can also access their member ID card when signed in.
