Introducing Horizon Care Management Plus℠

At Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ), we’re continually looking for ways to improve the health of our members and reduce costs for our customers.
That’s why, effective January 1, 2021, the Chronic Care Program will become Horizon Care Management Plus&8480;. This isn’t simply a name change. Horizon Care Management Plus combines the best of our Chronic Care and Case Management programs to create an integrated offering that connects your client’s covered employees and their dependents to personalized support for their acute and chronic conditions.
How it works
A designated Care Manager — who is a registered nurse — along with a team of licensed health care professionals, will coordinate care for members with the following high-risk conditions:
- Chronic Obstructive Pulmonary Disease (COPD)
- Coronary Artery Disease (CAD)
- Heart failure
Members will also have access to Specialty Programs, including:
- Behavioral health and Substance Use Disorder management in collaboration with Horizon Behavioral Health
- Cancer management
- Infertility, high-risk maternity and NICU management
- Infusion management
- Musculoskeletal condition management
Care Managers will collaborate with members’ doctors, as well as with external vendors, to intervene early, determine a care plan and help the members meet the goals of their plans. Care Managers also serve as advocates and educators, helping members learn about their conditions, navigate a complex health care system and maximize plan benefits.
