Improvements to the Small Employer Health Plus Plan

In less than six months, approximately 3,000 members have enrolled in Horizon Blue Cross Blue Shield of New Jersey’s new Small Employer Health Plus Plan. Thank you for helping make this comprehensive benefits package a success with your clients.

Based on your feedback, Horizon BCBSNJ revised the employer applications used in the small group market. We are pleased to announce the following improvements to the implementation process for the Small Employer Health Plus Plan.

  • Employers can now select their Horizon Dental coverage on the Small Employer Health (SEH) Medical application when purchasing medical coverage and the Horizon Health Plus Plan bundle together; a separate dental application is no longer needed.
  • The Horizon Vision application has been revised to omit information that has already been provided on the medical application.
  • For current medical plan groups purchasing the Horizon Health Plus Plan bundle, we have a combined Horizon Dental/Horizon Vision application.
  • USAble’s application now allows for more characters in the email address field.

The new forms are available at HorizonBlue.com/brokers.

The Small Employer Health Plus Plan makes it easier for your small employer clients to provide their employees with the care they need beyond accessing doctors and hospitals.
