Guiding Our Members to Better Health

Many Horizon Blue Cross Blue Shield of New Jersey members may have unique, ongoing health challenges. That's why Horizon BCBSNJ is working with MOBE®, a guided health solutions company, to offer personalized health services and coaching to eligible members.

Who is eligible?

Select members with unresolved health issues can get help improving their health outcomes through lifestyle changes, in combination with the medical care they already receive. Members who are eligible will be contacted by MOBE to participate.

What’s included

Eligible members receive information, guidance and support to help feel their best. Services include:

  • Being paired with a “MOBE Guide,” a health coach who will work with members through
    one-on-one conversations by phone or through the MOBE Health Guide app.
  • Guidance to improve a member’s overall quality of life, by addressing sleep habits, nutrition, exercise and stress management.
  • Access to pharmacists to discuss medication lists to optimize effectiveness.

There is no cost to eligible members who take advantage of these resources. MOBE does not replace appointments or care provided bythe member’s primary care doctor. Participation is strictly voluntary.

Members who participate are able to take control of their own health and move toward a healthier lifestyle.
