Identity Protection Services
Horizon offers identity protection services to eligible members through Experian, as an added feature of their health plan.
Personal information should stay that way -- personal.
Eligible Horizon members* are automatically registered for Experian's identity protection services. If a problem arises and a member needs identity repair assistance, they simply call Experian at 1-866-926-9801, Monday through Friday, from 9 a.m. to 11 p.m., Eastern Time (ET), and Saturday and Sunday, from 11 a.m. to 8 p.m., ET. A dedicated investigator will work with them to recover any financial losses, restore their credit and make sure their identity is returned to its proper condition.
For additional protection at no cost, members may also enroll in Experian IdentityWorks at any time. This gives them access to:
- Experian credit report at sign-up
- Credit monitoring Internet surveillance
- Up to $1 million identity theft insurance
- Lost Wallet
- Child monitoring
- Experian IdentityWorks ExtendCARE™
Experian Identity Restoration is another valuable service for our members. If a member becomes a victim of identity theft, a dedicated Identity Restoration Specialist from Experian will act as their guide and advocate from start to finish. Once contacted, an Identity Restoration Specialist will start the dispute process and help ensure that their identity returns to its pre-identity theft state.

*Horizon members enrolled in fully insured and self-insured group health plans, including the State Health Benefits Program (SHBP) and School Employees’ Health Benefits Program (SEHBP), and consumers enrolled in individual health plans on and off the Exchange, are eligible for identity protection coverage. Members enrolled in coverage through the Federal Employee Program® (FEP®), Medicare Advantage, Medicare Part D and Medicaid plans are not eligible for identity protection coverage.
