Our OMNIA Health Plans give you affordable access to quality doctors and hospitals, all for a competitive monthly premium.

With the three OMNIA options — OMNIA Bronze, OMNIA Silver, and OMNIA Gold — there’s no requirement to select a Primary Care Physician, or referrals needed to see participating specialists. You’ll also benefit from lower copayments and deductibles (and in some cases, no deductibles), when using OMNIA Tier 1-designated doctors, hospitals and other health care professionals.

Offers lowest monthly premium but highest out-of-pocket costs
On average, pays for 60% of covered medical expenses, while you pay 40%

New Jersey Medicare recipients are wonderfully unique. That's why, depending on what county you live in, we offer a variety of health plan options.

What is a deductible? A deductible is the amount you must pay for certain covered services each year before your health plan starts to pay. Once your deductible is paid in full, you usually pay only a copayment or coinsurance for any covered service.

What is a copayment? A copayment (or copay) is a flat fee that you may have to pay for covered services when you go to an in-network provider. Often, you’ll have different copay rates for different types of care.

What is coinsurance? Once your deductible has been met, you may have to pay coinsurance for certain covered services. You’ll pay a percentage of the fee that your health insurance company has negotiated with in-network providers.

What’s the difference between Tier 1 and Tier 2 in OMNIA plans? Horizon’s in-network doctors, health care professionals and hospitals are all designated Tier 1 or Tier 2. OMNIA members maximize benefits and lower out-of-pocket costs by using Tier 1 providers. You can choose providers in Tier 2, but you won’t get the same cost-saving opportunities.

How do I find out if my doctor is OMNIA Tier 1 or Tier 2? Visit our Doctor & Hospital Finder. Next, choose the type of care you’re looking for and the OMNIA plan, then enter your doctor’s name in the search field. The results page will display the doctor’s Tier 1 or Tier 2 designation.

Do OMNIA Health Plans have out-of-network benefits? Most OMNIA Health Plan members do not have out-of-network benefits for nonemergency care. However, if you or an eligible dependent will be outside New Jersey for 90 – 365 consecutive days, you may be eligible to apply for the Away From Home Care Program.

What is telemedicine? OMNIA Health Plan members have access to a convenient, confidential telemedicine service, Horizon CareOnline. Through this service, you can talk with a licensed, board-certified doctor via video, chat or phone, 24 hours a day, seven days a week, no appointment needed.

What does Maximum Out-of-Pocket (MOOP) mean? The MOOP is the most you’ll need to pay for covered services during your health plan year. Once you’ve spent this amount on deductibles, copayments and coinsurance (but not premiums) for in-network care, your plan pays 100% of covered costs.

What is an HSA? A Health Savings Account, or HSA, is a portable savings account to pay for out-of-pocket medical expenses. You can save up to a set amount of money each year, tax-free, and use it toward your deductible and other medical expenses your plan doesn’t cover.
