Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ) is responsible
for administering certain benefits on behalf of the New Jersey State Health Benefits
Program (SHBP). This SHBP Special Open Enrollment Period is your opportunity to review
the medical plan to which you have been assigned. The Special Open Enrollment Period will
be held for all eligible employees from January 28 through February 15, 2008. The new medical
plans offered during the Special Open Enrollment Period will become effective on March 29,
2008 for State employees paid by the State’s Centralized Payroll Unit, and April 1, 2008
for all other State and Local employees.
To make a change to the medical plan to which you have been assigned, you must submit an
Enrollment Application. Please refer to
"A
Guide to Choosing a SHBP Health Plan" for additional information.
Completed enrollment applications must be returned to your employer by February 15, 2008
(or the date established by your employer). Please do not send enrollment applications
directly to the SHBP or Horizon BCBSNJ.
